4-Week Training Plan
Get a personalized training plan delivered by email. All fields are required for the plan to be as useful as possible. This form is lengthy as it includes the client intake and PAR-Q form fields I require to create a plan that will be both safe and effective for your goals.
IF YOU COMPLETE YOUR FORMS AND IT IS DETERMINED THAT I AM UNABLE TO PROVIDE YOU WITH A PLAN YOUR FEES WILL BE REFUNDED.
This workout plan is a great starting point but ultimately how well you implement the plan will impact your success.
Please submit payment within 24 hours of completing the form. I review forms once payment is received.
If you have any of the following conditions I am not the best person to provide you with a training plan as I don’t do in person or live personal training and cannot monitor you. Heart disease, high blood pressure, diabetes, stroke.
If you are seeking a trainer to provide one to one coaching, I do not offer that service. I do offer live ‘Office Hours’ 1 to 2 times per month as part of my Office Hours member subscription.
